The very name of these health insurance providers, health maintenance organizations, carries with it the idea for which they were created. Work Within The System

This doctor will then become the "gatekeeper" and he or she is the one that must approve whether or not you see another physician - for any reason.

Other doctors may be outside of the HMO system, but may work in contract with the organization. They are referred to as individual practice associations (IPA's), and they do provide regular medical care for HMO patients. Besides your monthly premium, there may also be a small copay that needs to be made with each doctor visit, or other expense. Usually your HMO membership card serves like a credit card, so that all billing is automatic. You also need to be careful about going outside the system without their preapproval - they probably will not pay for it.
